At the entry portal wait for a npc that says the "pass word". In say-say the pass word and you will teleport in. It is the reverse line from one of my favorite movies.

What I do is make another skill bar above it and drag the highest useable reaction available there. Use the other bar as an indicator. Just a work around until it is all fixed.

In the skull head there are Red, Blue, and Yellow councilors. They drop a corresponding mask. Once you have all 3 go outside to crow on the pillar and click on one. He will be summoned. Difficult fight ensues.
