If you start a quest from an item, the dialog box will not close on accept and if you have an NPC targeted when you right click the quest item, it will hail the npc on Accept. This sseems to me to be related to the bug you encounter when interacting with dialog windows via world object click.

Which quest? This is controlled in some way by the value of unknown6 but I would need to specifically see what is going on. There are quests starting from items that I've done in the past that didn't have this issue so it is possible.

Assuming this is about inventory items and not just game objects, this does not happen in regards to the quest "Bloodhowler Plans" (Id 2087 ) with the Ragged Parchment (ID 621969) item that starts it.

If we do run into problems, I had to add a flag to force the quest window to close after accept and not try to open dialog when adding the command .quest offer. I can easily add a parameter in Lua's DisplayQuest() function that when specified as true will force the window closed with no dialog attempt. I believe there is just something off in the quest database entry that will likely solve it.
I'm also pretty sure this setting was not stored on the server. I think that when an npc has additional quests that have not been accepted, unknown6 is set to hail after you accept a quest, so you can immediately choose from the others. I think it is set to close the dialog when there are no more quests, or you are not currently in a dialog with an npc. I wrote some code to determine if a character is actually in dialog, but it didn't work, because technically they are not by the time it goes to the quest offer window. I will go back sometime and see if I can create some sort of "in-dialog" flag so we can hopefully get over the hump on this particular unknown6 issue.

Updated unknown 6 and 7A and now the window closes just fine also it does not hail the npc if you have one targeted.
