Fails to hit.
or

Succeeds roll.
PC: *Rolls to Dodge/Block/Parry*
PC: *Dodge successful*
PC: *Reaction activation.*
or
PC: *Dodge Failure*
PC: *Mitigation with Resistances & AC is now calculated.*
or
PC: *Dodge Success*
or in someones case with multiple mitigation skills...
PC: *Dodge Failure*
PC: *Roll to Block*
PC: *Block Successful!*
or
PC: *Dodge Failure*
PC: *Block Failure*
PC: *Roll to Parry*

or most commonly,

It could be handled different ways, however, Vanguard was patched so that even on a miss, dodge/parry/block can be triggered.
