Bug Form
I wasn't sure where to put this, so I hope this forum is okay for it.
I used to do QA for Funcom on Age of Conan and The Secret World. We had a form for GM's to fill out bug reports when reported by users. I thought maybe this could come in handy here. Might not really come in handy yet, but it also might not hurt to get used to filling them out like this. (The devs said it made things much easier for them.)
Fun Fact: Funcom GM's weren't allowed to say the word 'bug', we actually had to call it 'issue', which became very confusing when TSW game updates were called "Issues" as well.

Here's my example for the Chath Encounter that I wrote for the OVF.
•Bug: Chath Encounter/Magicborn Teleporter Malfunctions (GYG has reminded me of THIS one).
•Bug Type: Gameplay
•Quests/Items involved: Griffon's of a Feather: Chp 5 Creepy Cadavers
•Synopsis: The teleportation into the chamber/area of play doesn't always work.
•Description of Bug: In order to engage Chath and Magicborn, your group/raid needs to get teleported into the chamber. Quite often the characters are left behind. We are not exactly sure what causes it, but in our experience we've had greater luck if: The chunk is recently reset. Characters do not wear any illusions. No pets are spawned. Characters remove Sorc ward buffs. We suspect that characters might be reflecting or even resisting the 'spell' that is cast on them to teleport them inside.
•Steps to Reproduce it:
1. Get a standard group together for Chath run, Level: 50+ characters. Tank, Healer, 4DPS. Go to Heroes' Plain Chunk, to the start area.
2. Spawn the quest on at least one member of the group. (Quest name is Griffon's of a Feather: Chp 5 Creepy Cadavers)
3. Have the quested group member hail the portal and click the dialogue. Observe if characters make it in. If they all make it in, go in and have the group wipe (We suspect this might have something to do with it as well)
4. May need to do this a few times, especially if the chunk was reset/woke up. Eventually players will be left out.
•Other Notes: The fact that these events have a timer makes this even more irritating, if half your raid gets left out on Magicborn you have to wait 30 minutes to try again - only to have it happen over and over.

I developed a fairly extensive Bug Tracker for EQ2, which I have already ported over to VGO - it integrates directly with the /bug report in-game (for EQ2, not yet for VGO). My RL career (20+ years) is SQA, so you can bet your bottom I have adequate bug tracking Unfortunately, getting people to use it is another story.
Aside from the in-game bug report, the web portal will have a dialog like this (already developed) I am hoping to use the VGOPlayers.com integration to link bug reports to active accounts, so players can manage their own bug reports as well (see them, update them, cancel them, etc). Though this is a long ways off, because pretty much everything is a bug right now... all 2 features

This made me smile. I was always curious why they released new issues. And to release issues to resolve issues is just very ... fun for your head and tongue.

•Synopsis: use of .rift or .speed commands in /say channel seem to bug my chat channels (maybe)
•Description of Bug: (get ready for a wall) Yesterday I logged in and .rift to Keejay Cove (player housing area Qalia) from Tanvu. When I got there i was running rather slowly. I got some help to rev up my run speed but the command was not working. After having attemped to use the .speed 3000 command I noticed similarly my .rift command had stopped working. Soon I basically lost all ability to run, or even camera angle change in game. my only remaining functions were use of chat channels. I relogged. After this I had no use of chat channels and New Telon channel was no longer listed in my chat channels list (similarly all channels in my channels list had entirely disappeared), no run speed change and still no use of .rift. I completely restarted my computer at this point. After logging back into the game I had use of .speed and .rift commands but no use of channels. New Telon channel had reappeared in my channels list. I also noted that /who all command stopped working and I could also not send /tells. *Note .speed 3000 is damn nifty.
•Steps to Reproduce it: I did do a fair bit of running around and have even visited other housing areas and didn't notice any variations in my experience. I did however zone to (86) Cliffs of Ghelgad and on approaching the area of the Kraken on the beach there was extreme lag. (this would be the part where my husband sighs and says "FPS!", hes a software test engineer, the snob) anywho something was practically gluing me in place. I managed to make it to the zone and crashed zoning. Today I logged in and fell through the world and crashed again to desktop a few times. Finally logged in in the water and began to have the same problem with the .rift and .speed commands not working. After a few more crashes to log in and me manually restarting the client I was able to log in and .rift to Khal. After I got there .speed started working. I still can't use any chat channels, although I can see New Telon in my chat window and my chat log. Also no /who all. *side note I did finally get use of my emotes /dance etc. (those werent working last night right before my first crash).

I would like to ammend this report to include the following: I had to reboot because I crashed while zoning attempting to swim to Skawlra Rock (102) in an effort to escape Keejay Cove. (Going to develop an island phobia soon I can feel it!) This time while crashing / zoning I couldn't tab out to desktop.

Hi Moon, John has very recently (I think just today?) implemented a bug tracker system! Would you be able to try it out with the bug(s) you talk about above?

Heh It's actually been there for months, we just had no bugs since we're sooo alpha, I wasn't really taking any (because people will generally report bugs about things not even implemented - and nothing is implemented) I just posted that so Jak could see, we take our bugs seriously!
Definitely give it a try though. I think it works
